When seeing a participating general dentist, payments
are guaranteed according to our Schedule of Benefits.
You are entitled to choose any participating dentist
on our plan. Dental offices are located throughout
the United States.
If
you require dental treatment by a Network Specialist
or are referred to a Specialist by a Participating
Dentist you will receive the discounted fee listed
on the "Fee Schedule." This includes
Orthodontics, saving you a considerable amount
off of the usual and customary fee.
